February 2026 the NSW Ombudsman made findings of maladministrations under the Ombudsman’s Act due to a culture of “ No capacity to allocate (NCTA) ROSH risk of significant harms and desk top decision making only resulting in several coroners reports on children, babies,toddlers and teens who were well known eg 13 ROSH reports previously to DCJ. Prior to that the auditor General found 370k unallocated cases and prior to that in 2018 the Tune Report documented the same workplace culture concerns within this statutory child protecting office. Children’s commissioners positions need review as does the office of the children’s guardian and working with children ( wcc) beaurecracies, who only do marketing campaigns. Privatisations of Foster Care agencies has been a disaster as reported in the 2026 no capacity to allocate report.
